CentraCare Health is seeking a Director of the St. Cloud Hospital at Home (HaH) program to build and scale an integrated care delivery model that enables hospital-level care at home.

You will oversee nursing operations, clinical operations, logistics, virtual care, and partnerships, ensuring safe, high-quality care and regulatory compliance. The role provides leadership to nursing, intake, field operations, and remote patient monitoring, manages budgets and contracts, and collaborates with

Qualifications

• Bachelor's degree in nursing required.
• Three years of clinical nursing experience in hospital or home health care.
• Two years of progressive leadership experience at supervisor level or higher.
• Minnesota RN licensure required.
• Demonstrated leadership skills including management, communication, strategic development, organization and goal setting.

Responsibilities

• Lead nursing, clinical operations, logistics, virtual care and remote patient monitoring for HaH.
• Oversee contracts, workforce management, budget oversight, and regulatory readiness.
• Coordinate HaH with physicians and other clinicians to ensure patient-centered care.
• Develop and implement work plans aligned with system strategies to optimize care at home and foster collaboration.
