Job summary

CommQuest Services, Inc. in Canton, OH, is seeking a Director of Community-Based Services to lead a team of case managers across social work, behavioral health, and community services, ensuring access to resources for clients.

You will oversee referrals, supervise staff, and coordinate with providers, funders, and the criminal justice system; maintain records and report outcomes. This role offers growth, meaningful impact, and a partnership with Great Minds Fellowship.

Qualifications

• Master's degree in Clinical Counseling or Social Work required.
• LPCC-S / LISW-S licensure in Ohio required.
• 7+ years in Operations with at least 3 years supervisory experience.

Responsibilities

• Lead and manage a team of case managers and support staff.
• Accept referrals, assign staff, and guide on complex cases.
• Collaborate with providers, funders, and community resources to ensure access to services.
• Maintain accurate case records, document assessments and outcomes, and report program results.
• Perform clinical record keeping and provide client education.
• All other duties as assigned.

Education
