Job summary

Ascension St. Mary's in Milwaukee, WI is seeking a Case Management leader to shape and drive operational strategies within the program. This role directs services, develops care plans, and partners with physicians to reduce variation in care, ensuring evidence-based practice and quality outcomes.

Qualifications

• High school diploma or higher with 2 years of experience; master's preferred.
• 5 years of experience required; 2 years in leadership or management.

Responsibilities

• Formulates and implements operational strategies and initiatives in the case management program.
• Directs and evaluates all services provided, including care plan development, service level determination, and complaint management to achieve performance and quality control objectives.
• Provides specialized expertise, ensures resource alignment with system-wide initiatives, and updates appropriate parties on case management strategies, initiatives, outcomes and issues.
• Partners with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care.

Education

High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required. Master's degree preferred.
