Job summary

Ascension Health in Milwaukee seeks a qualified social work/case management professional to assess patient needs, develop care plans, and coordinate the interdisciplinary plan from admission to discharge. You will advocate for patient rights, provide resources and education, and collaborate with leadership and care transition teams to ensure timely plan reevaluation and optimal outcomes.

This role requires Wisconsin licensure eligibility; preferred CCM credential; and a background in guiding

Qualifications

• Licensed Clinical Counselor or Social Worker credential from the Wisconsin boards, obtained prior to hire date or transfer date.
• Preferred Case Manager credential (CCMC).

Responsibilities

• Develops care plans for patients based on assessment findings and needs/preferences.
• Assesses complex emotional, psychological and physical issues to tailor social work support.
• Advocates for patients’ needs and ensures the medical system respects their rights and preferences.
• Maintains dialogue with leadership and care transition teams to implement and reevaluate care plans.
