Job summary

Saint Mary of Nazareth Hospital is seeking a Director of Case Management to lead the development of staff and systems for a comprehensive program in Chicago, IL. The role involves supervising case managers, social workers, discharge planners, and utilization teams, while aligning services with patient and family needs and the hospital mission.

Qualified candidates should have an MSW and LCSW, plus at least five years in case management and strong analytical, data-review, and computer skills.

Qualifications

• Licensed clinician in your state; Grandfathered prior to April 1, 2015. Minimum 5 years' post graduate of an accredited school of Social Work for Licensed Clinical Social Worker.
• Minimum 5 years' experience in a Case Management position.
• Analytical ability for problem identification and evaluation of data/statistics from ongoing review.
• Experience and knowledge in basic to intermediate computer skills.

Responsibilities

• Develop staff and systems to operate a comprehensive Case Management Program.
• Provide leadership and supervision to case managers, social workers and coordinators/discharge planners, utilization review coordinators and technicians.
• Assess needs, plan, and design services aligned with hospital mission and patient/family needs.
• Integrate and coordinate services using continuous quality improvement tools.
