Job summary

Citizens Options Unlimited is seeking a Direct Care Assistant for an Intermediate Care Facility in New York. The role involves assisting residents with activities of daily living, hygiene, feeding, and mobility, while supporting independence and safety. AMAP certification allows medication administration under physician orders.

You will participate in goal-based programs and interdisciplinary team meetings. High school diploma or equivalent is preferred, and a NYS driver’s license is required.

Qualifications

• Six months experience working with DDI population preferred.
• AMAP certification is an added qualification when administering medications.

Responsibilities

• Assist residents with personal hygiene and daily living activities to maintain independence.
• Follow goal books to implement resident-specific programs aimed at independence and life goals.
• Assist with diet adherence to maintain ideal weight range and portion control.
• Provide medication administration when AMAP certified and per doctors' orders.
• Maintain resident safety through supervision and use of adaptive equipment.
• Supervise residents to ensure safety and well-being at all times.
• Support recreational activities and social engagement through arts, games, and physical activities.
• Attend interdisciplinary team meetings to discuss progress and share information.
