Job summary

Sevita is seeking a Direct Support Professional to provide daily living assistance in residential settings. You will support individuals with social activities, health maintenance, medication administration, transportation, and community integration per individualized plans.

In this non-exempt hourly role (full-time or part-time), you will document progress, coordinate with nurses and supervisors, and help ensure safe, respectful care in a supportive team environment.

Qualifications

• Six months of experience in human services preferred.
• Working knowledge of computers.
• Must be 18 years of age.

Responsibilities

• Deliver support per individual service/program plans, including socialization and behavioral development.
• Assist with daily activities: personal care, housekeeping, recreation, transportation, shopping.
• Direct Support: assist with socialization, health maintenance, medication administration, and community orientation.
• Records: document progress and review logs; maintain confidentiality.
• Management of assets: make cost-effective purchases within the individual’s budget.
• Relationships: maintain professional relationships with individuals, families, guardians and case managers.
• Reporting: report abuse/neglect and medical/behavioral incidents according to policy.
• Rights: protect confidentiality and individual rights.
• Appointments: accompany to medical appointments and relay information to providers.
• Meal preparation: assist with meals according to planned menus.
