Job summary

Sevita is seeking a Direct Support Professional to provide day-to-day assistance to individuals in residential settings. You will implement individualized service plans, support socialization, monitor health, and help with medications as directed.

This hourly, non-exempt role involves helping with daily living activities, documenting progress, and maintaining confidentiality. A high school diploma and 6 months human services experience are preferred; must be 18+ and have basic computer skills.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other daily living tasks.
• Accurately document progress and activity; maintain confidentiality.
• Maintain professional relationships with individuals, families, guardians and case managers.
• Report any abuse, neglect or incidents per Sevita policies and applicable requirements.
• Respect individuals' rights and follow universal precautions.
