Job summary

Hope Haven Inc. is seeking Direct Support Professionals to assist clients with personalized goals, ensuring physical, mental, emotional, and social needs are met through compassionate care and person-centered support.

As a DSP you will implement service plans, help clients with daily activities, coordinate transportation, document progress, and promote safety while fostering independence within the community.

Qualifications

• High school diploma or GED/HSED preferred
• Good written and verbal communication skills and basic computer skills
• Ability to exercise sound judgment in decisions
• Ability to complete/pass required medication administration course
• Valid Class C driver’s license
• Clearance from background checks as required by service standards
• Reliable transportation
• Proof of state minimum liability insurance coverage

Responsibilities

• Provide support and training to individuals as identified in the service plan and assure individual’s physical and mental needs are cared for, including hygiene, bathing, toileting, escort, transition time, and mealtime assistance.
• Teach self-control and self-management skills as per departmental training.
• Complete documentation requirements (client logs, charts) by end of shift or within 24 hours with supervisor approval.
• Support individuals in developing and carrying out a daily schedule of activities tailored to meet their needs.
• Assist individuals in arranging appointments and transportation and ensure documentation is completed.
• Promote personal safety and follow all safety procedures.
