Job summary

Hope Haven Inc. is currently seeking Direct Support Professionals (DSP) to assist clients with achieving their personal goals and daily needs. DSPs provide support across physical, mental, and social areas to help individuals live more independent lives.

Responsibilities include implementing service plans, assisting with daily activities, documenting care, coordinating transportation, and maintaining safety during shifts. Relevant certifications and a valid driver’s license are required.

Qualifications

• Good written and verbal communication skills and basic computer skills.
• Ability to exercise sound judgment in decisions.
• Ability to complete/pass required medication administration course.
• Valid Class C driver’s license is required.
• Requires background checks per service standards.
• Reliable transportation.

Responsibilities

• Support clients to meet physical, mental, emotional, and social needs as per individual goals.
• Provide support and training per service plan, including hygiene and daily living tasks.
• Teach self-control and self-management skills per departmental training.
• Complete client logs and charts within required timeframes.
• Help clients develop and carry out daily activity schedules.
• Arrange appointments and transportation with proper documentation.
• Promote safety and follow all safety procedures.
