Job summary

Open Arms Care is seeking a Direct Support Professional to support individuals with ID/DD in Nashville. This full-time/part-time role involves implementing person-centered activities, documenting progress, and coordinating with families and the care team.

Candidates must have a high school diploma or GED, a valid driver’s license, CPR certification within 30 days, PCM certification within 60 days, and be able to lift up to 50 pounds. Training and benefits are provided.

Qualifications

• High school diploma or GED preferred.
• Must have valid driver’s license.
• CPR certification within 30 days of hire is required.
• Certified in Professional Crisis Management (PCM) within 60 days of hire is required.
• Ability to lift repetitively a minimum of 50 pounds.

Responsibilities

• Represent Open Arms Care in a positive and professional manner in community meetings and events.
• Provide day-to-day organization and activities that promote an organized, person-centered environment.
• Make timely decisions on best practices, quality assurance, risk management and problem-solving.
• Coach, teach and mentor individuals in home management and independence.
• Engage clients in ongoing treatment and implement ISP-based programs as per CMS regulations.
• Coordinate transportation and appointments, prepare and review required documents.
• Document client participation and progress in training objectives to teach independent living skills.
• Communicate with inter-disciplinary teams and family members to maintain positive relationships.
