Job summary

Mentor Community Services, part of the Sevita family, seeks a Direct Support Professional to work in community settings, collaborating with clinical staff to implement therapeutic and behavioral plans and help individuals served live as independently as possible.

You will assist with transportation, daily living skills, meal prep, med administration, and support a range of developmental goals while maintaining dignity and respect for those served.

Qualifications

• High School Diploma or GED required.
• Must be at least 18 years old.
• Six months of experience in human services preferred.
• Valid driver’s license in good standing.
• Access to a registered vehicle with proof of insurance.
• Successful clearance of background checks.
• Reliable, responsible attitude.
• Compassionate approach and commitment to quality in everything you do.

Responsibilities

• Transportation and support out in the community: outings/field trips, medical appointments, and shopping.
• Assist with daily living skills including meal preparation and assist with medication administration.
• Provide guidance on developmental goals: community participation, budgeting, exercise, nutrition, and vocational responsibilities.
• Support personal hygiene, grooming, bathing, toileting.
• Assist individuals to redirect agitated or aggressive behaviors.
