The role is non-exempt, hourly, and may be full-time or part-time. Responsibilities include direct support, documentation, health maintenance, and community integration within residential settings and program

Qualifications

• High school diploma or equivalent preferred.
• Six months experience in human services preferred.
• Must be 18 years of age and have a valid driver’s license.
• All state-required trainings and background checks completed.

Responsibilities

• Deliver support per individual service plans, including socialization and health maintenance.
• Assist with daily activities: personal care, housekeeping, transportation, and community integration.
• Accurately document progress and maintain confidentiality.
• Maintain relationships with individuals, families, guardians, and case managers.
• Safely manage medications if assigned and report incidents per policy.
• Participate in safety and training activities.
