Job summary

Mentor Community Services, part of Sevita, seeks Direct Support Professionals to provide compassionate, person-centered care in residential settings. You will execute service plans, assist with daily living activities, and document progress while maintaining confidentiality and rights of individuals served.

This non-exempt role emphasizes teamwork, safety, and adherence to health-care protocols, including medication assistance and transportation when needed.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service/program plans including socialization and health maintenance.
• Assist with daily activities: personal care, housekeeping, recreation, transportation, community integration, shopping.
• Accurately documents progress and maintains records/confidentiality.
• Make cost-effective purchases within the individual's budget and preferences.
• Maintain healthy relationships with individuals, families, guardians and case managers.
• Report abuse, neglect, medical and behavioral incidents as required.
• Respect rights and maintain confidentiality; practice universal precautions.
• Attend orientation and ongoing training; participate in monthly staff meetings.
• Follow scheduling expectations and communicate with supervisor; cover shifts when needed.
• Participate in safety policies; report hazards and assist in safety training.
• Transport individuals to community activities and drive safely when required.
• Perform housekeeping tasks and light maintenance; ensure safe environment.
• Work environment: program location and community outings.
• Physical requirement: heavy work; handle lifting as required.
