Job summary

Mentor Community Services, a part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities. We’re seeking a part-time Direct Support Professional to provide direct support within a single program, typically serving 4–6 individuals in residential settings, with focus on daily living activities and community integration.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per service plans across socialization, health, meds, and behavior development.
• Assist with daily activities: personal care, housekeeping, recreation, transport, shopping.
• Document progress and reviews records; maintain confidentiality.
• Make cost-effective purchases within the individual's budget and preferences.
• Maintain professional relationships with individuals, families, guardians, and case managers.
• Report abuse, neglect, and medical/behavioral incidents per policy.
