Sevita, through Mentor Community Services, seeks a Direct Support Professional Mobile to assist individuals in community-based settings, closely collaborating with clinical staff to implement therapeutic plans. Qualifications

• High School Diploma or GED required.
• Must be minimum of 18 years of age.
• Six months of experience in human services preferred.
• Valid driver’s license in good standing and access to a registered vehicle with proof of insurance.
• Successful clearance of background checks and a reliable, compassionate attitude.

Responsibilities

• Provide transportation and support in the community, including outings, medical appointments, and shopping.
• Assist with daily living skills, meal preparation, and medication administration.
• Support development goals in areas like community participation, budgeting, exercise, nutrition, and vocational tasks.
• Assist with personal hygiene, grooming, bathing and toileting.
• Help individuals redirect agitated or aggressive behaviors.
