Job summary

Mentor Community Services, part of the Sevita family, is seeking a Direct Support Professional Mobile to support individuals in community settings. You’ll work closely with clinicians to implement plans that promote independence and well-being.

The role includes transportation, daily living support, and helping clients reach developmental goals. A driver’s license and vehicle are required, with full-time benefits available for eligible staff.

Qualifications

• High School Diploma or GED is required.
• Must be at least 18 years old.
• Six months of experience in human services preferred.
• Valid driver’s license in good standing required.
• Access to a registered vehicle with proof of insurance required.
• Successful background checks and a compassionate approach.

Responsibilities

• Provide transportation and support in the community for outings, medical visits, and shopping.
• Assist with daily living skills including meal preparation and medication support.
• Guide on goals in areas like community participation, budgeting, exercise, nutrition.
• Support living skills such as hygiene, grooming, bathing, and toileting.
• Redirect agitated or aggressive behaviors to maintain a safe environment.
