Qualifications

• High school diploma or GED required.
• Massachusetts LPN or RN license with current license.
• 1–3 years of related experience.
• Experience with Department of Developmental Services (MR/DDS) preferred.

Responsibilities

• Assess and coordinate healthcare services for individuals, ensuring timely follow-up and communication with SHCS staff.
• Assist and transport individuals to medical appointments per DDS standards.
• Document nursing interventions, medical updates and care changes in EHR.
• Maintain and review medication and treatment orders; monitor for side effects and report concerns.

Direct Support Professional III - LPNPay: $40.00/hour base rate; $41.00/hour Cape Cod rateSchedule: Sunday 7am-9pm, Friday 7a-10a and 2pm-9pm, Saturday 7am-9pmThe Direct Support Professional III - LPN is a direct service position providing individual medical support and education to individuals in assigned programs who advocate for all medical needs of assigned program participants focusing on optimal health for all individuals.
