Job summary

Seven Hills Foundation is seeking a Direct Support Professional III - LPN to provide direct medical support and education to individuals in assigned programs, advocating for all medical needs and coordinating healthcare services. This full-time role requires MA LPN or RN licensure, 1–3 years of related experience, and a valid MA driver's license.

Work hours: Mon–Fri 3pm–9pm with a monthly 2-hour staff meeting.

Qualifications

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) with current Massachusetts license.
• 1–3 years of related healthcare experience.
• High school diploma/GED/HiSET.
• Valid driver’s license (Massachusetts).

Responsibilities

• Assess and coordinate healthcare services for individuals, ensuring timely follow-up and communication with SHCS staff.
• Assist and transport individuals to medical appointments in accordance with DDS standards.
• Document nursing interventions, medical updates, and care changes in Electronic Health Records (EHR).
• Maintain and review medication and treatment orders; monitor for side effects and report concerns to appropriate personnel.
• Support ISP implementation by understanding and monitoring relevant care components (e.g., behavior plans, hygiene, self-medication).
• Train staff on medication policies and procedures.
• Ensure health records are accurate and compliant with DDS regulations.
• Communicate medical updates and treatment plans to residential and day program staff.
• Collaborate with Health Care Director or program RN for supervision and clinical guidance.
• Participate in staff meetings and mandatory in-services.
• Uphold agency policies and maintain professionalism, confidentiality, and respect at all times.
