Job summary

Continuum of Care is seeking a Direct Support Professional in Connecticut to support individuals with intellectual disabilities in living independently and engaging with community activities. You will provide face-to-face assistance with daily living, personal care, and related tasks, while following individualized plans and behavioral programs.

You’ll coordinate transportation, attend team meetings, and help track progress in electronic records.

Qualifications

• Bachelor’s or Associate degree preferred; HS diploma or equivalent with related experience accepted.
• CT driver’s license with clean driving record required.
• Direct Support Professional certification preferred.
• CPR & First Aid, PMT and DDS/CoC certifications completed or in progress.
• Computer skills to navigate electronic records and documentation.
• Availability to work evenings, weekends, and overnights as required.

Responsibilities

• Establishes rapport and positive working relationships with individuals while maintaining ethical standards and boundaries.
• Demonstrates sensitivity and commitment to treating individuals with respect and dignity.
• Provides hands-on support for ADLs, personal care, and activities promoting healthy living.
• Delivers services per IP and positive behavioral programming where applicable.
• Encourages use of natural supports and community participation while ensuring safety.
• Provides companionship and problem-solving discussions.
• Supports individuals to attend routine medical, dental, and other appointments.
• Arranges transportation to access community resources.
• Contributes to Individual Plan development and supports aspirations and goals.
• Works with community providers for quality care.
• Inputs data in Therap or other designated electronic systems; tracks daily data.
• Attends staff and other designated meetings; completes other client care tasks.
