Job summary

Continuum of Care is seeking a Direct Support Professional to assist individuals with intellectual disabilities in their Meriden, CT home. You’ll help with daily living activities, promote independence, and support community involvement with dignity.

Duties include hands-on care, documenting in Therap or similar systems, implementing individual plans, and coordinating with the care team. Weekend shifts pay a $1/hour differential; evening availability is required.

Qualifications

• Experience in human services or related field.
• Ability to work evenings, weekends, and overnight as required.
• Valid Connecticut driver's license with clean record.

Responsibilities

• Establish rapport and maintain professional boundaries.
• Provide hands-on support for activities of daily living.
• Assist individuals to participate in community activities and appointments.
• Document data input daily and complete IP-focused notes.
• Attend staff meetings and contribute to care planning.
