Job summary

Continuum of Care is seeking a Direct Support Professional in Southington, CT. You will support individuals with intellectual disabilities in daily living, personal care, and community integration, empowering them to live independently.

The role requires evening, weekend, and overnight availability as needed; CPR/First Aid certifications and a valid CT driver’s license are preferred. Data entry in Therap and collaboration with care teams are essential.

Qualifications

• Bachelor’s or Associate’s degree preferred; HS diploma with relevant experience considered.
• Valid Connecticut driver’s license with clean driving record.
• CPR & First Aid, PMT certifications preferred.

Responsibilities

• Provide face-to-face hands-on support for activities of daily living and personal care.
• Assist individuals to live independently and participate in community activities.
• Document daily data and IP-related information in Therap or designated systems.
• Attend staff meetings and support IP plan development.
