Job summary

Continuum of Care seeks a Direct Support Professional to assist individuals with intellectual disabilities in living independently and participating in community life.

The role emphasizes respectful, dignified care, daily living support, personal care, and engagement in social activities. The position requires flexible hours including evenings and weekends, and involves data entry using Therap and other systems.

Qualifications

• Bachelor’s or Associates preferred; HS diploma acceptable with related experience.
• Valid Connecticut driver’s license with clean record.
• Certification as Direct Support Professional preferred.
• CPR & First Aid, PMT, and required DDS/CoC trainings completed.
• Computer skills to navigate agency software and email.
• Evenings, weekends, and overnights availability.

Responsibilities

• Provide face-to-face hands-on support enabling independent living and activities of daily living.
• Deliver services per individual plan and behavioral programming; encourage social engagement.
• Support medical, dental, and other appointments; assist transportation to resources.
• Input data into Therap or designated systems; track and report daily IP data.
• Attend staff and designated meetings; perform other client care tasks as assigned.
