Job summary

Continuum of Care in Connecticut is seeking a Direct Support Professional to support individuals with intellectual disabilities in their own homes and community settings. You will assist with activities of daily living, transport to appointments, and facilitate engagement in social and community activities, while maintaining dignity and respect.

Weekend shifts and detailed data entry into Therap are required.

Qualifications

• High school diploma or equivalent; relevant human service experience preferred.
• Connecticut driver’s license with clean driving record.
• CPR & First Aid, PMT, and all required certifications through DDS and CoC.
• Computer skills to navigate agency electronic records, access email, and complete documentation.
• Available to work evenings, weekends, and overnights as required.

Responsibilities

• Establishes rapport and positive working relationships with individuals while maintaining professional ethical standards and boundaries.
• Provides face‑to‑face hands‑on support enabling individuals to maintain living in their apartment/home, including activities of daily living and personal care.
• Delivers services in accordance with individual plan (IP) and positive behavioral programming, as applicable.
• Provides input in Individual Plan development supporting individuals to achieve aspirations and goals.
• Works with community providers for quality care of individuals.
• Responsible for assigned data input with Therap or designated electronic systems.
• Tracks and reports daily data for identified IP teaching, ADL strategies, behavioral, and medical needs.
