Job summary

Easterseals Blake Foundation is seeking a Direct Support Professional to provide in-home and community-based assistance to individuals with intellectual, developmental, and physical disabilities. You will support daily living activities, safety, and participation in community activities, while following individual support plans and maintaining accurate records.

Excellent customer service, calm under pressure, and ability to work in diverse environments are essential.

Qualifications

• High School Diploma or GED preferred.
• Three (3) months of caregiving experience preferred.

Responsibilities

• Assists individuals with personal care and hygiene needs.
• Performs housekeeping tasks and daily living activities.
• Provides transportation and community-based support.
• Guides community participation, budgeting, exercise and nutrition.
• Administers medications or assists with self-administration.
• Maintains individual files and follows ISP.
• Reports incidents and assists with incident forms.
• Performs on-the-job training and supports members in employment.
