Job summary

Continuum of Care in Southington, CT is seeking a Direct Support Professional to assist individuals with intellectual disabilities in living independently and engaging with their community. The role emphasizes dignity, personal care, and support with activities of daily living.

You will document care activities, follow IPs, coordinate with providers, and participate in meetings. Evening, weekend, and overnight shifts are required; competitive wages and comprehensive benefits included.

Qualifications

• Valid Connecticut Driver’s license with clean driving record required.
• Computer skills required to navigate agency electronic records, access email, and complete documentation.
• Available to work evenings, weekends, and overnight shifts as required.

Responsibilities

• Establishes rapport and positive relationships with individuals while maintaining ethical standards.
• Provides face-to-face hands-on support for activities of daily living and personal care.
• Delivers services according to individual plans and behavioral programming as applicable.
• Encourages participation in social and community activities and ensures personal safety.
• Maintains data input and daily reporting in designated electronic systems.
