Job summary

Continuum of Care seeks a Direct Support Professional to assist individuals with intellectual disabilities at home and in the community. You will provide hands-on support for daily living, personal care, and social activities, while promoting dignity and independence.

Role includes documenting in electronic systems, following IP plans, and coordinating with community providers. Evening/ weekend shifts may be required.

Qualifications

• Bachelor’s or Associate preferred; high school diploma with relevant experience acceptable.
• CT driver’s license with clean record is required.
• Certification as Direct Support Professional preferred.
• CPR & First Aid, PMT and DDS/CoC requirements completed.
• Computer skills to navigate electronic records and documentation.
• Evenings, weekends, and overnights may be required.

Responsibilities

• Establishes rapport and professional relationships with individuals.
• Treats each person with respect and dignity.
• Provides face-to-face hands-on support for ADLs and personal care.
• Delivers services per individual plan and behavioral programming.
• Facilitates participation in social and community activities.
• Supports medical, dental, mental health appointments and transportation.
• Maintains data input in Therap or other electronic systems.
• Attends staff and other designated meetings.
