Job summary

Imagine the Possibilities, Inc. seeks a Direct Support Professional to provide compassionate, person-centered care in Home and Community-Based Services settings in Iowa.

You will partner with a care team to implement individualized goals and support individuals in daily living, mobility, transportation, and community participation. The role requires strong communication, reliability, and willingness to learn.

Responsibilities

• Develop individualized goals with HCBS Supervisor and Programmer.
• Provide direct support focused on teaching skills and promoting independence.
• Document all services and interventions in Setworks daily, in a timely and compliant manner.
• Deliver hands-on care in home and community settings, including daily living assistance and mobility support.
• Administer and document medications following the six rights and agency policies.
