Job summary

Evergreen Life Services is seeking a compassionate Direct Support Professional to maintain a healthy, safe, and therapeutic environment for individuals served, following their care plans.

You will assist with daily activities, medication, transportation, budgeting, and home maintenance, while documenting progress and reporting any changes to the team. A strong English reading/writing ability and reliable transportation are required.

Qualifications

• Must read, write and communicate in English; able to read instructions, keep records, and write reports.
• Valid driver’s license preferred for insurance purposes.
• Meet and maintain all requirements of applicable state agencies.
• Submit to and pass a drug screen and criminal background check.
• Ability to utilize basic computer skills (e.g., data entry, approving timesheets).
• Prior experience as a direct support worker or in a caregiving capacity.
• Must be at least 18 years of age (non-driver) or 21 years of age (driver).

Responsibilities

• Maintain a healthy, safe, and therapeutic environment per care plans.
• Assist individuals served with medication and treatment plans as needed.
• Assist individuals served in care and cleaning of their homes, including reporting maintenance needs.
• Assist individuals served with money management, including budgeting, banking, and shopping.
• Assist individuals served with daily life activities such as meal preparation, reading, use of telephone, transport, housekeeping, and personal hygiene.
• Make food and non-food purchases for the home when requested.
• Prepare meals.
• Maintain the home and grounds as required.
• Complete all required documentation following policies.
• Implement behavioral programs as trained and requested.
• Assist individuals served with goals in their plan of care.
• Identify potential behavior triggers and defuse or redirect them for safety.
• Provide transportation for individuals served in company or personal vehicle.
