Job summary

Evergreen Life Services is seeking a Direct Support Professional to maintain a healthy, safe, and therapeutic environment for individuals served. You will follow individual care plans and provide support across daily living activities, meals, transportation, and documentation.

Responsibilities include assisting with medication, budgeting, and community services, while adhering to state agency requirements and organizational policies.

Qualifications

• Excellent reading/writing and verbal communication in English.
• Valid driver’s license and insurance requirements.
• Pass state agency checks and background screen.
• Basic computer/data-entry skills.
• Previous direct support or caregiving experience.
• Must be 18+ (non-driver) or 21+ (driver).

Responsibilities

• Maintain a healthy, safe, therapeutic environment.
• Assist with medication/treatment plans as needed.
• Help with home care tasks and reporting maintenance needs.
• Assist with money management, budgeting, banking, shopping.
• Support daily living activities and personal hygiene.
• Prepare meals and purchase home supplies as requested.
• Ensure home/grounds are well maintained.
• Complete documentation per policies and state regulations.
• Implement programs and support plan goals.
• Assist with transportation to appointments and activities.
