Job summary

Evergreen Life Services seeks a Direct Support Professional to maintain a healthy, safe, and therapeutic environment for individuals served, following care plans and state guidelines. This is a part-time role with responsibilities across daily living activities, transportation, budgeting support, and documentation.

Applicants should be able to read and write in English, pass a background check, and have a valid driver’s license if required.

Qualifications

• Must read, write and communicate verbally in English.
• Valid driver’s license (per agency requirements) preferred.
• Meet state agency requirements and be in good standing with abuse/neglect registry.
• Submit to drug screen and criminal background check.
• Basic computer skills for data entry and timesheets.
• Prior direct-support or caregiving experience preferred.
• Must be at least 18 (non-driver) or 21 (driver).

Responsibilities

• Ensure health and welfare of individuals served.
• Assist with medication and treatment plans as needed.
• Help with home care, maintenance reporting needs.
• Assist with money management, budgeting, banking, shopping.
• Assist with daily activities: meals, reading, phone use, community services, housekeeping, personal hygiene.
• Make food and non-food purchases for the home when requested.
• Prepare meals and maintain home/grounds as requested.
• Complete required documentation per state agencies and policies.
• Implement programs and assist with goals in care plans.
• Identify triggers and defuse or redirect for safety.
• Provide transportation in company or personal vehicle as needed.
