Evergreen Life Services is seeking a Direct Support Professional I to provide compassionate in-home and community support for individuals served. You will help implement care plans, promote safety, and encourage independence in daily activities.

The role involves assisting with medications, homemaking, budgeting, transportation, and documenting progress in accordance with state agencies and Evergreen policies.

Qualificações

• Read, write, and communicate verbally in English; able to read instructions and write reports.
• Valid driver’s license is preferred for insurance purposes.
• Meet state agency requirements, including good standing with the abuse/neglect registry.
• Submit to and pass a drug screen and criminal background check.
• Ability to utilize basic computer skills (data entry, timesheets).
• Prior experience as a direct support worker or in a caregiving capacity.
• Must be at least 18 years of age (non-driver) or 21 years of age (driver).

Responsabilidades

• Ensure the health and welfare of the individuals served.
• Assist individuals with medication and treatment plans as needed.
• Assist individuals in care and cleaning of their homes and report maintenance needs.
• Assist individuals with money management, including budgeting, banking, and shopping.
• Assist individuals with daily life activities such as meals, reading usage, telephone use, community services, housekeeping, clothing care, self-care, and hygiene.
• Make food and non-food purchases for the home when requested.
• Prepare meals.
• Ensure the maintenance of the home and grounds is done as requested.
• Complete all required documentation following state agencies and Evergreen policies.
• Implement behavioral and other programs as trained and requested.
• Assist individuals with goals outlined in their plan of care documents.
• Identify potential behavior triggers and defuse or redirect them as needed for safety.
• Provide transportation for individuals served in company or personal vehicle as requested.
