Qualifications

• Minimum 18 years of age.
• High school diploma or GED required.
• At least 2 years’ experience with individuals with disabilities.
• Valid driver’s license required.

Responsibilities

• Provide hands-on support and guidance to individuals to maintain health services delivery.
• Assist with daily living tasks and personal care.
• Schedule appointments and transportation for individuals to and from the community.
• Establish and maintain medical records following policies and procedures.
