Job summary

OneWell Health Care seeks a dedicated Caregiver / Direct Support Professional in Reading, PA to support individuals with IDD in home and community settings. The DSP provides hands-on, person-centered support, helps with daily living activities, health monitoring, and community participation while promoting independence and well-being.

This field-based role requires flexibility, reliable transportation, and adherence to care plans and HIPAA.

Qualifications

• High school diploma or equivalent.
• Preferred: prior experience in caregiving, home health, or direct support for individuals with IDD.
• Experience in community-based or residential settings.

Responsibilities

• Provide direct, person-centered DSP support in home and community settings per ISP/IPP.
• Assist with daily living activities, meal prep, mobility, and medication assistance as authorized.
• Document observations, activities, and outcomes; follow care plans and regulatory standards.
