The role emphasizes person-centered care, adherence to ISP/IPP plans, HIPAA compliance, and collaboration with an interdisciplinary team. Field-based, flexible schedule; driving may be required.

Qualifications

• High school diploma or equivalent.
• Prior caregiving or direct support experience with IDD preferred.
• Ability to follow written care plans and documentation guidelines.

Responsibilities

• Assist with personal care and daily living activities.
• Support meal preparation, nutrition, mobility, and medication aid as authorized.
• Implement behavior support strategies and document outcomes.
• Maintain health, safety, and emotional well-being; participate in trainings and meetings.
• Travel to homes and community sites; flexible shifts.
