De Band Global LLC is seeking a personable Direct Support Professional (DSP) to provide direct care to recipients in Assisted Living, Natural Home, and the Community. The DSP will support independence per Plan of Care.

The role involves assisting with daily hygiene, coordinating services, and documenting care while collaborating with families and guardians. In-person work with flexible shifts and a friendly team environment is offered.

Formación

• High school diploma or equivalent is required.
• Minimum 1 year caregiving experience is required.
• English language proficiency is required.
• Valid Driver's License is required.

Responsabilidades

• Provides support services as outlined in the Plan of Care and/or support plan and any behavioral health protocols.
• Teaches and assists with daily personal hygiene, household chores, and recreational activities.
• Collaborates with the team to coordinate services and monitor approved POCs and support plans.
• Develops and maintains positive relationships with families and guardians.
• Coordinates transportation to medical appointments using a personal vehicle or agency-provided transport.
• Monitors health and reports concerns to the Program administrator and Care Coordinators.
• Completes required documentation accurately and timely according to agency standards.
• Performs other tasks within the scope of the position and in the best interest of recipients.
