Qualifications

• High School Diploma or GED is required.
• Preferred: minimum 1 year experience with individuals with intellectual and developmental disabilities.

Responsibilities

• Facilitates a customized community-driven schedule based on individuals' wishes and goals.
• Assists individuals to engage in their community to develop natural supports and peers.
• Supports independent living skills per the individual's goals.
• Coordinates with families and teams to address concerns.
• Provides transportation to planned activities to access the community.
• Maintains safety and cleanliness of the homes and documents incidents and goals daily.
• Participates in medical appointments and reviews nursing status.
