Job summary

Responsibilities

• Provide one-on-one assistance with daily living activities, maintaining dignity and privacy.
• Develop individualized care plans with clients and teams based on needs and goals.
• Support community access and participation in social, recreational, and vocational activities.
• Apply positive behavior support strategies to address challenging behaviors.
• Monitor health, report changes, and document observations for healthcare teams.
• Keep accurate records of progress, incidents, and daily activities.
• Ensure safety and respond to emergencies when needed.
