You will follow Person-Centered Plans, document activities in the electronic system, and work with families and the care team to promote safety, wellness, and independence.

Qualifications

• Experience with person-centered planning.
• Ability to follow care plans and document activities.
• Comfort with basic household tasks and meal preparation.

Responsibilities

• Provide direct care and support per PCP and planning team.
• Teach and assist daily living skills.
• Assist with medications as prescribed.
• Ensure safety and cleanliness of the home.
