Job summary

Hope Haven, Inc. is seeking Direct Support Professionals (DSP) to assist clients with intellectual disabilities or chronic mental health conditions.

You will provide individualized support to help meet physical, mental, emotional, and social needs, while promoting independence and participation in daily activities. Responsibilities include assisting with daily living tasks, documenting care, coordinating appointments, and ensuring safety.

Qualifications

• High school diploma or GED preferred.
• Good written and verbal communication skills and basic computer skills.
• Ability to exercise sound judgment.
• Ability to complete/pass required medication administration course.
• Valid Class C driver’s license required.
• Clearance from background checks as required by service standards.
• Reliable transportation.
• Provide proof of state minimum liability insurance coverage.

Responsibilities

• Help support clients according to individual goals, including meeting physical, mental, emotional, and social needs.
• Provide support and training per service plan; assist with hygiene, bathing, toileting, escort, transition, and mealtime tasks.
• Teach self-control and self-management per departmental training.
• Complete client logs and charts within 24 hours with supervisor approval.
• Support clients in developing a daily activity schedule tailored to their needs.
• Arrange necessary appointments and transportation; ensure documentation is completed.
• Promote safety and follow all safety procedures.
