Job summary

Hope Haven, Inc. is seeking Direct Support Professionals to assist clients with intellectual disabilities or chronic mental health conditions. You will support daily activities, maintain goals, and ensure physical, mental, and social needs are met.

Responsibilities include training, documentation, transportation assistance, and safety adherence. A valid driver's license and background checks are required, with comprehensive benefits and growth opportunities offered.

Qualifications

• High school diploma or GED/HSED is preferred.
• Good written and verbal communication skills.
• Ability to exercise sound judgment.
• Ability to complete medication administration course.
• Valid Class C driver's license required.
• Clearance from background checks as required by service standards.
• Reliable transportation.
• Proof of state minimum liability insurance coverage.

Responsibilities

• Support clients according to individual goals and service plans.
• Provide support and training, including activities of daily living.
• Teach self-control and self-management skills per training.
• Complete client logs and charts within 24 hours with supervisor approval.
• Support daily schedule of activities tailored to client needs.
• Assist with appointments and transportation; ensure documentation.
• Promote safety and follow all safety procedures.
