Job summary

Hope Haven, Inc. is seeking Direct Support Professionals to assist clients with intellectual disabilities or chronic mental health conditions. You will support individuals to meet personal goals across physical, mental, and social needs.

Responsibilities include following service plans, assisting with daily activities, and ensuring proper documentation. A valid driver's license and background checks are required; a range of health and retirement benefits is offered.

Qualifications

• High school graduate or GED/HSED preferred.
• Good written and verbal communication skills and basic computer skills.
• Ability to exercise sound judgment in decisions.
• Ability to complete medication administration course.
• Valid Class C driver’s license required.
• Clearance from background checks as required by service standards.
• Reliable transportation.
• Provide proof of state minimum liability insurance coverage.

Responsibilities

• Support clients based on individual goals, ensuring physical, mental, emotional, and social needs are met.
• Provide support and training per the service plan, including hygiene, bathing, toileting, escort, meals.
• Teach self-control and self-management skills per departmental training.
• Complete documentation (client logs, charts) by shift end or within 24 hours with supervisor approval.
• Support individuals in developing and carrying out a daily activity schedule tailored to their needs.
• Arrange necessary appointments and transportation and ensure documentation is complete.
• Promote personal safety and follow safety procedures.
