Qualifications

• Minimum high school diploma or GED required.
• 18+ years old and legally eligible to work.
• Valid driver's license in good standing and reliable transportation.

Responsibilities

• Provide assistance with Activities of Daily Living (ADLs) per plan.
• Administer prescribed medications after training.
• Safely transport individuals in company vehicles for community integration.
• Complete documentation in the electronic health record (EHR) timely.
• Prepare meals and snacks per individual/family requests.
• Support goal completion per individualized support plan.
• Engage with individuals in day programs and the community.
• Uphold individual rights and HIPAA privacy at all times.
• Adhere to policies and maintain professional boundaries.
• Maintain facility cleanliness and report maintenance needs.
• Proficient with technology; stay awake and alert during shifts.
• Perform physical tasks as required (lifting/transfers).
• Other duties as assigned.
