Job summary

AHRC Nassau is seeking Direct Support staff to help individuals with intellectual and other disabilities participate in daily living in their homes and communities. You will support self-care routines, health, safety, and household chores while promoting independence and engagement in daily activities.

Responsibilities include assisting with meals, transportation to medical appointments and social outings, and collaboration with the inter-disciplinary team to implement treatment plans and goals.

Qualifications

• High School Diploma or equivalent preferred.
• Must have a valid NYS driver's license and maintain qualified driver status.
• Ability to understand and communicate written and oral directives/information.

Responsibilities

• Provide ongoing guidance and support to individuals to promote participation in daily routines and independence.
• Support individuals in self-care, health, safety, and household activities to maintain wellbeing.
• Assist individuals with transportation to appointments and community activities.
• Participate in team meetings and follow agency policies and procedures.
