Job summary

OneWell Health Care in Olanta, PA is seeking a compassionate Direct Support Professional (DSP) to provide hands‑on, person‑centered care to individuals with IDD in home and community settings.

You will implement individualized service plans, assist with daily living, health monitoring, and community participation while promoting independence and dignity. This field‑based role requires flexible scheduling and adherence to HIPAA and company policies.

Qualifications

• High school diploma or equivalent is required.
• English proficiency is required for effective communication.
• Valid driver’s license may be required depending on assignment.
• Experience in caregiving preferred.
• Ability to follow care plans and document accurately.

Responsibilities

• Provide consistent, respectful, high‑quality support in accordance with each individual’s care plan.
• Assist with personal care and daily living activities.
• Support meal preparation, nutrition, mobility, and medication assistance as authorized.
• Implement behavior support strategies; monitor health, safety, and emotional well‑being.
• Promote independence, stability, and community participation; assist with transportation to appointments or outings.
• Participate in trainings and team meetings; maintain accurate documentation; adapt to staffing needs.
