Job summary

Cape Abilities Farm in Hyannis, MA seeks a Direct Support Professional to implement ISP objectives and provide one-on-one personal care to individuals with developmental disabilities. You will collaborate with caregivers and staff to promote independence, community involvement, and safety.

The role requires a high school diploma with an associate degree preferred. Prior direct care experience and a valid MA driver’s license are preferred.

Qualifications

• High school diploma or equivalent required.
• Associates degree or additional education in related fields preferred.
• 1 year of direct care or human services experience preferred.

Responsibilities

• Implements ISP objectives and behavior plans as defined in service agreements.
• Prepares ISP paperwork and reports according to procedures and deadlines.
• Provides training in personal care to individuals who require support.
• Provides one-on-one supervision to individuals to ensure safety.
• Completes reports and maintains communication with families and staff.
• Adheres to routines and schedules to meet individual needs.
• Supports scheduling and attendance at social and leisure activities.
• Provides transportation to appointments and activities.
• Respects the individual’s basic rights, well-being and safety.
• Follows supervisory directives and established protocols for one-on-one care.
• Attends required meetings and trainings.
