Job summary

Cape Abilities in Hyannis, MA is seeking an Individual Supports Professional to help participants develop independent living, social, and recreational skills and stay safely engaged in the community. Qualifications

• High school diploma or equivalent required.
• Associate’s degree or additional education in psychology, social work, nursing, or special education preferred.
• 1 year of direct care or human services experience preferred.
• Ability and willingness to provide hands-on personal care.
• Must be able to communicate effectively in verbal and written form with coworkers and supported individuals.
• Valid driver’s license in Massachusetts required.
• Successful completion of state and Agency mandated trainings upon hire required.

Responsibilities

• Implements ISP objectives and behavior plans as defined on established service agreements.
• Prepares and forwards all required ISP paperwork and reports according to procedures and deadlines.
• Provides training in personal care to individuals who require support.
• Provides one-on-one supervision to individuals to ensure safety as assigned by treatment goals and plans.
• Completes written reports including program objective documentation and incident reports; maintains communication with families and staff.
• Adheres to established program routines and schedules to meet individual needs.
• Provides assistance and support to individuals with scheduling and attending social and leisure activities.
• Provides transportation to appointments and social activities.
• Ensures that the individual’s basic rights, well-being and safety are respected.
• Follows supervisory directives and protocols when providing one-on-one support.
• Attends required meetings and trainings.
• Serves as mandated reporter for suspected abuse/neglect to DPPC or Elder Abuse Hotline.
