Responsibilities

• Participates in Circle of Support and helps develop goals.
• Supports ISP and BSP achievement.
• Documents progress using agency systems.
• Completes required training and applies skills daily.
• Provides safety, independence, and daily living support.
• Assists with meals, personal care, and transportation as needed.
• Maintains accurate records and participates in inspections.
