Summary
The Direct Support Professional works on an interdisciplinary team that delivers superior quality of service and provides appropriate activities in a safe and healthy environment within the home and community setting.
Essential Duties & Responsibilities
- Assists clients in basic living skills and daily personal tasks such as showers, dressing, housekeeping, grocery shopping, preparing and planning meals, and financial budgeting.
- Provides transportation to and from activities and appointments as needed; a valid Arkansas Driver’s License is required.
- Maintains good communication via appropriate means (facility logbook, staff meeting, etc.) regarding clients’ needs and progress toward their plan of care by the end of shift.
- Accurately completes all documentation required by FCC, including Behavioral Reports, Incident Reports, Complaints Forms, Personal Care Work Sheets, Requisitions, Maintenance Forms, Progress Reports, Monthly Attendance Logs, and other required documentation.
- Follows and completes necessary documentation and records for FTA vehicles, maintaining all mileage/billing forms in compliance with Medicaid guidelines.
- Reports incidents and/or emergencies to supervisors or delegated authorities immediately.
- Assists in the formulation, management, revision, and follow-through of the client’s Plan of Care, ensuring treatment is carried out and good health, safety, and security are prioritized.
- Follows proper medication management guidelines and assists in monitoring and documenting these procedures.
- Organizes and carries out recreational activities for clients.
- Demonstrates good role modeling, maintaining a professional appearance in accordance with FCC dress code and appropriate actions.
- Treats clients and their families with dignity and respect, reflecting FCC’s mission in all interactions.
- Assists in maintaining FCC facilities, grounds, and vehicles, keeping them clean, orderly, and functioning.
- Attends necessary training and continues education.
- Provides proof of liability insurance and maintains current coverage.
- May be required to undergo periodic driving tests and demonstrate proficiency in operating a motor vehicle, including passenger vans. Employees may also drive their personal vehicle at times and must maintain adequate liability coverage.

- Maintains confidentiality of all information received regarding FCC and its clients, in compliance with FCC’s Non-Disclosure Policy and HIPAA regulations.
Quality Assurance & Compliance
- Adheres to the Code of Conduct and Compliance Investigation Procedures as outlined in new employee training.
- Maintains and completes consumer progress notes on a daily or as needed basis using prescribed forms.
- Keeps consumer records updated weekly, monthly, quarterly, and yearly, filing necessary information as required by funding sources.

Qualifications & Requirements
- High school diploma or GED; or two+ years’ experience working with developmentally disabled individuals.
- Valid driver’s license.

Ability to read and comprehend simple instructions, short correspondence, and memos; write simple correspondence; and present information effectively to clients and team members in one-on-one and small group settings.
Mathematical/Computer Skills
Ability to add, subtract, multiply, and divide using whole numbers, common fractions, and decimals; compute rate, ratio, and percentage; and interpret bar graphs.
Reasoning Ability
Application of commonsense to carry out instructions provided in written, oral, or diagram form; interpretation of various instruction formats including schedules.
Physical Demands
