Summary
The Direct Support Professional works on an interdisciplinary team that delivers superior quality of service and provides appropriate activities in a safe and healthy environment within the home and community setting.
Responsibilities
- Assists clients in basic living skills and daily personal tasks (shower, dressing, housekeeping, grocery shopping, preparing and planning meals, and financial budgeting).
- Provides transportation to and from activities and appointments as needed; a valid Arkansas Driver’s License is required.
- Maintains good communication via appropriate means (facility logbook, staff meeting, etc.) regarding clients' needs and progress toward their plan of care by the end of shift.
- Accurately completes all documentation required by FCC (behavioral reports, incident reports, complaint forms, personal care worksheets, requisitions, maintenance forms, progress reports, monthly attendance logs, and other required documentation).
- Follows and completes necessary documentation and maintenance of records for FTA vehicles; maintains all mileage/billing forms in compliance with Medicaid guidelines.
- Reports incidents and/or emergencies to supervisors or delegated authorities immediately.
- Assists in the formulation, management, revision, and follow-through of the client’s Plan of Care to ensure treatment is carried out and good health, safety and security are prioritized.
- Follows proper medication management guidelines and assists in monitoring and documentation of these procedures.
- Organizes and carries out recreational activities for clients.
- Exemplifies good role modeling for clients through appropriate physical appearance and adherence to FCC dress code.
- Treats clients and their families with dignity and respect; represents FCC’s mission consistently.
- Assists in the maintenance of FCC facilities and grounds, keeping them clean, orderly and functioning.
- Assists in the maintenance of FCC vehicles, ensuring cleanliness and regular preventive maintenance.
- Attends necessary training and continues education.
- Provides proof of liability insurance and keeps it current.
- Might be required to undergo periodic driving tests and demonstrate proficiency in operating a motor vehicle, including passenger vans; employees may need to drive personal vehicles at times, maintaining adequate liability coverage.
- Maintains confidentiality of all information received regarding Friendship Community Care, Inc. and the clients, in compliance with the company Non-Disclosure Policy and HIPAA policies.
Quality Assurance & COMPLIANCE
- Follows the Code of Conduct and Compliance Investigation Procedures instructed during new employee training.
- Maintains and completes consumer progress notes daily or as needed on prescribed forms.
- Maintains consumer records weekly, monthly, quarterly, and yearly; files information in consumer records as needed and updates as prescribed by funding sources.

Qualifications
- High school diploma or general education degree (GED); or two+ years of experience working with developmentally disabled individuals.
- Valid driver’s license.
Language Skills
- Ability to read and comprehend simple instructions, short correspondence, and memos.
- Ability to write simple correspondence.
- Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.
Mathematical/Computer Skills
- Ability to add, subtract, multiply, and divide using whole numbers, common fractions, and decimals.
- Ability to compute rate, ratio, and percentage, and to draw and interpret bar graphs.
Reasoning Ability
- Application of commonsense to carry out written, oral, or diagram instructions.
- Interpretation of a variety of instructions furnished in written, oral, diagram, or schedule form.
Physical Demands
- Requires use of hands for handling or feeling objects, with frequent standing, walking, reaching, sitting, climbing, stooping, kneeling, crouching, or crawling.
- May occasionally lift/move up to 100 pounds.
- Specific vision requirements include close vision, color vision, depth perception, and the ability to adjust focus.
